Chronic Care Management
Chronic care management encompasses the oversight and education activities conducted by health care professionals to help patients with chronic diseases and health conditions such as high blood pressure, heart failure, and atrial fibrillation learn to understand their condition and live successfully with it. The work involves motivating patients to persist in necessary therapies and interventions and helping them to achieve an ongoing, reasonable quality of life.
WHY WOULD I NEED IT?
Studies have shown that patients enrolled in Chronic Care Management programs enjoy a better quality of life and survive longer when diagnosed with serious chronic illnesses. It supplements your care between scheduled visits with the goal of reducing sick visits and to prevent you from being hospitalized.
HOW IT WORKS
Meet with Dr. Wang or Ginger to determine if you would be a candidate that would benefit from the program.
Discuss things like goals, objectives and expectations. All of which are personalized for you.
Follow through at home with what was discussed during planning
Speak with your nurse about your current condition and determine if medication/ care adjustments are needed. Frequency and schedule is determined in the planning phase
